Test Description
Designed to be completed by the parent or guardian of children ages 5-12 years, the Children's Problems Checklist contains 202 items that cover topics including anxiety, self-image, relationships with peers, school performance, communication, attention, harmful behaviors, values, personal hygiene habits, and health. It is useful as an intake survey to identify relevant problems, establish rapport, and provide written documentation of presenting problems and is helpful when designing an assessment battery. Administration takes about 20 minutes.
