Designed as an intake aid for mental health professionals, the Mental Status Checklist for Adolescents is a 174-item survey that covers topics familiar to those who work with adolescents ages 13-17 years. It gathers information about the presenting problems as well as personal information, academic performance and attitudes, developmental status, legal issues, health habits, hobbies, family relationships, and more. It also includes areas for behavioral observations and clinical impressions.
