Designed as an interview tool or intake aid, the Mental Status Checklist for Children is a 153-item survey that covers topics familiar to mental health professionals who work with children ages 5-12 years. It gathers information about the child’s presenting problems as well as personal information, academic performance and attitudes, social and developmental status, legal issues, health habits, hobbies, family relationships, and more. It also includes areas for behavioral observations and clinical impressions.
